Is it possible for a NB Bakerloo Line train to enter the NB platform at Paddington at line speed (which I think is 20mph due to the sharp curve into the station), whilst another train is either parked to the N side of the crossover (to be reversed into the SB platform), or whilst another train is traversing the crossover into or out of the SB platform?

This move is identical to Elephant, Lambeth and Picadilly. Same crossover set up, even same use of signal numbers... BR4 BR10, BP4 BP10, BD4 BD10 etc.
Lambeth is the only one that gets used reguarly - the others normally only in a degraded service.
At Lambeth, a train can pull into the northbound platform at full line speed (30) whilst a train is at at BR4 - the wrong road shunt signal. The shunt move cannot be performed until that approaching train is at a standstill though. ie. at a danger signal in or before the station. The points just won't throw.
I can't give you a conclusive answer regarding Paddington, but I don't see why it wouldn't be the same.

I would be very surprised if a full line speed approach was possible at Piccadilly if the n/b line was occupied or the crossover was reversed. Given the presence of the five speed control signals, the high curvature of the platform, and the fact that the stopping mark is definitely within the crossover fouling point, I would expect that any approaching train would get held at one of the n/b home signals.

The inner home will remain at danger if a train is shunting north to south at both Paddington and Picc. As far as i am aware, the crossover at Lambeth is a lot further up and a train can berth fully in the NB if one is sitting at the London Rd/SB platform wrong road shunt in the NB tunnel. The NB stopping mark for this move is actually nearer Waterloo than Lambeth if memory serves.

My ignorance is playing up again so I will ask whether it is possible to do a NB-SB reversal at Piccadilly Circus via the tunnel? Is there a signal?

Yes, BP4. u.t.l. says that it is a colour light signal too. Correct on both counts. And the "temporary" 5mph speed restriction has been on there for as long as I can remember (and some!!). You're bang on about the stopping mark fouling the crossover at Picc too. I stand corrected One of those things I've seen (thousands of times), but I've never seen it... lol
