But, it says here:
How could this have happened?
To me, I imagine someone being hit maybe because they were on the line as the train came into the station?

Retreiving an item of luggage, say mobile or small bag? Not forgetting that there is a 10mph limit into this station,
Maybe they fell just as the train approached, slipped, frightened by a noise from the train or another [horn being sounded]
or they were just drunk?

While of course guessing:
Perhaps walking along the track (report does state Liverpool Street area). Fairly common when people are a little drunk and have missed their last train.
One-unders at Terminus stations are quite rare (AFAIK) so probably not a deliberate act.

There's also a possibility that they were leaning over close to the edge of the platform for some reason. Seems to be a bit of a favourite for teenagers to stick limbs out at the last minute to either touch the front of the train or try to spook the driver.
