Last time I travelled though Waterloo (3/10/22), neither travolator was working and there didn't seem to be any "out of service" notices. Are there out of service long term and if so what is the problem?

Well, you learn something new every day. I'll include them on my next trip to town.

While "Trav-O-Lator" is a trademark, it appears that travolator is becoming generic (similar to escalator) in casual conversation.

The travelator hasn't worked for quite a while - or does so sporadically, as in one side is on whilst the other is off. I haven't seen them both in operation at the same time for quite a while. For most of this year when I've had to use them it's been over a stationary walkway. The last time I walked across them, some of the panels clunked and moved slightly under my feet, which I must say was quite unnerving.

Yes, they don't seem to have been in good order in quite some time - at least over year that regularly they've been stationary as I don't have cause to use them as much as I used to. About as reliable as the escalators at Tower Gateway DLR.

BTW, in the US licensed elevator/escalator mechanics are rather dear, I imagine the same is true in the UK. (Why? seems like in the last 20-30 years fewer people go into the trades.) Some transit agencies employ their own instead of relying on contractors.

I vaguely recall reading about some sort of tragic incident which (I think) involved a maintenance engineer and the Jubilee line moving walkways.

Very difficult to carry out major work.
There is only limited space between the moving walkways.
Erecting a hoarding around one of the walkways to enable overhaul would severely restrict the flow of passengers.
The length of the passage is not that long so it may be that leaving them in a fixed position is the sensible option.

Yes, a maintenance engineer was killed in September 2019. The latest news I can find is from the following year and the incident was still being investigated then. www.bbc.co.uk/news/uk-england-london-54112345The walkways have been in service since then though, so that can't be the reason.

There is now a hoarding up around one of them, so passengers in one direction (Northern/Bakerloo towards Jubilee if I'm remembering right, and assuming the working walkway doesn't get reversed at some point during the traffic day) are now having to walk in the space between the walkways.
There's also now something official about this on the TFL closures page, the current estimate being that at least one will be out of service until June 2023.
