When there is a signal failure and the train is stated as “working under failure conditions” what does that actually mean?

That the signalling has a fault but still safe with the appropriate procedures put in to still run a service but of course with delays.
If your referring to today then the vast majority of the signal failures have been track circuit failures due to flooding.

There is quite a few depending what sort of area the signalling has failed its very complex and I'm sure the drivers on here will agree its drummed into them to follow every step by step correctly
