For example, the Romford to Upminster line which isn't physically connected to the rest of the London Overground network, though neither are the Lea Valley lines aside from out of station interchanges, or TOC's that operate a line outside of its normal geographical area.
What other examples of this are there? Past or present.

The Liverpool & Manchester Railway wasn't connected to the rest of the network for a period of time.
You might get better answers if you ask on a general railway forum, rather than here.

Historically, all the narrow gauge railways included in GWR, LMS, SR and LNER in 1923.

Northern's Barton on Humber - Cleethorpes route is isolated from the rest of the Northern network during the week - all other trains at Cleethorpes are TransPennine or East Midlands, which have to be used to connect with the nearest other Northern services at Scunthorpe or Lincoln.
However, on Saturdays Northern does run three (?) trains from Sheffield to Cleethorpes and back.
Once TfL Rail takes over the Paddington local services, I believe the Greenford-West Ealing shuttle will not connect with any other GWR service.

The idea of a TOC was unknown until 1993. Until then we had railway companies. There were a number of companies (e.g. the owner of the Forth Bridge) who owned railways but didn’t operate trains but I can’t think (maybe you will) of any company that only operated trains and didn’t also own (or long lease, in the case of the Shropshire & Montgomeryshire) a railway. Some railways also had running powers over a neighbouring line but that’s not the same thing.

The two funicular railways in Hastings are not connected to each other and both are operated by the local council I believe, and likewise the two in Bournemouth.
The two portions of the Great Orme Tramway are owned and operated by the same organisation but are not physically connected.

I can’t think (maybe you will) of any company that only operated trains and didn’t also own a railway. What about Pullman services? The Surrey Iron Railway and (I think) the Stockton & Darlington were originally "open access", (based on the same model as the canal companies) so the operators on those lines didn't own the railway. And private owner wagons have always been with us.

The two sections of Merseyrail (loop and link) are physically separate except for a connection at Liverpool Central that carries no passenger services. Does that count? The Wirral and Northern Lines are connected by two interchanges (Moorfields and Liverpool Central), I thought we were only including OSIs (Out of Station Interchange)
