Acton Central. Bollo Lane near South Acton has two a few hundred yards apart. Private entrance road to Neasden Depot (visible from the public road bridge) Entrance road to Beckton Depot (visible from the public road, but security get snotty about you photographing)

If that's how it was a few years ago then it's particularly nice as it doesn't have the conventional siren but a singly struck bell (ding ding ding not dingalingalingaling) . Mind you, it's probably been modernised now.

You could have a look at carto-metro. Light Rail crossings are shown in blue, and heavy rail in red. It does seem to be public roads only though, as some of the above mentioned, mainly those on private roads, such as neasden, aren't shown carto.metro.free.fr/cartes/metro-tram-london/

The only foot crossing on the Underground is just north of Amersham - which means that although the track is owned and maintained by the Underground, only Chiltern Railways' trains pass over it.
There used to be a lot of level crossings on the LNER out to Epping, but these were closed as part of the conversion works when the Central line took over in the 1940s.

This picture from The Woodford Historical Society shows Snakes Lane, Woodford Station's level crossing. My Dad remembers it well. The crossing was replaced by a pedestrian subway only, leaving the poor old road traffic with no choice other than to use Broadmead Road or go via Roding Valley to get from one side of the road to the other! The buildings on the West side are all still intact although owned by different institutions.
