On a number of LU trains - the C stock were a very good example of this, but I think it's still possible on some of the trains still in service - the doors could be opened before the train had actually come to a complete stop.
I'm wondering exactly what was/is required on these trains for the doors to be opened - presumably the receiver has to be lined up with the CSDE loop and everything? What's the maximum speed at which this could be done?
But I'm also wondering about any safety measures that were used. Could a train open up by this means and then miss the stopping mark? Also is there anything to actually stop a train from continuing on through the station - doors open - without ever coming to a halt at all. Obviously it's difficult to see how this could be done by accident, but I was merely wondering about any failsafes that were/are used where this arrangement is in place.

S Stock must come to a complete stand before the doors can be opened. What's the maximum speed at which this could be done? 6mph. Could a train open up by this means and then miss the stopping mark? Also is there anything to actually stop a train from continuing on through the station - doors open - without ever coming to a halt at all. In your example there would be nothing to prevent a train (i can only speak upon the Stock i've worked) from opening doors under 6mph, within the CDSE loop and continuing to roll through the station, (motoring could not be obtained), EXCEPT the professionalism of the driving staff employed.

2009 stock often open just as they are coming to a stop.

hopefully nothing like this will happen ClickyI would hope that travellers in London rather than choosing to film the episode and publish it online might active the nearest alarm to ensure the safety of their fellow passengers. After all, they wouldn't want to film a "snuff" video. Video converted from embedded to a link so that members can see the context and commentary around in.

hopefully nothing like this will happen ClickyUsed to happen all the time with the very old S Bahn trains in Berlin and also the old red double deckers in Sydney. Travelled on both of them with open doors between stairs. People coped with it because it had always been like that. Nowadays we have fifty kittens if something moves a millimetre out of place. Doors on the older Metro and RER stock in Paris also opens before trains stop if people press the button / lift the handle. I'll admit an opening door on high floor double deck RER stock coupled with low platform heights is slightly scary because of the distance you could fall at speed.

Although sometimes you may also be worried about stuck on a train. I remember years ago visiting the Vienna U-Bahn. In order to get off the train you had to open the doors yourself but the handles were often stiff
