That does confirm that part of the train was in the tunnel (so that the CSDE would need to be overridden to open the doors), and that the train operator was unaware of what was happening (and so wouldn't immediately know of the need to open the doors).
The picture looks rather more like Stratford than Holland Park though!

Yes looks like the RAIB have cause for concern here - no surprise there eh! "the RAIB considers that there was potential for injuries to occur because the train was heavily loaded and a large number of people were prevented from leaving the train for almost four minutes after it stopped."

There is no reason for passengers to not be able to open doors in an emergency at a station. Understandable, but to back up Chris M's point and put it into seemingly plain english, if part of a train is in a tunnel, a train can't open it's doors. You have just been proved wrong by the RAIB "About two and a half minutes after the train stopped, station staff reached the platform and opened one pair of doors from the outside. Approximately one minute later, the Train Operator opened all the doors adjacent to the platform"

There is a clue a few lines later: The Train Operator was not aware that a fault with the brakes on one car in the train was generating smoke and fumes.
A reasonable inference would be that passengers became aware of the smoke and fumes and operated the alarm for that reason, but it's not being offered explicitly at this stage. If a statement were available on the lines of "I operated the alarm and this is why" then the report would be worded accordingly. In any event, it's not the key point. The key point is whether the evacuation could have been handled differently under the circumstances.

The RAIB have launched an appeal for witnesses to this incident as detailed in this Forum Announcement. Forum Staff have liased with the RAIB and would encourage any member or reader of this Forum witness to the incident to contact the RAIB as detailed in the announcement in order to improve passenger safety.
