Just reading that article and the three "related" articles linked by bbc news, they all have somthing in common:
Passengers struck by Tube train: ""Safety is our top priority and we have an excellent safety record, we are investigating this incident, which is very rare.""
Tube safety call over red signals: "A spokeswoman for LU said: "Safety is our top priority and London Underground has an excellent safety record.""
Tube trains 'came close to crash': "A Transport for London spokesman said: "Safety is LU's top priority and we have an excellent safety record.""
Oh dear, I think someone needs to stop using copy and paste in their press releases or get a new spokesperson.

It's called 'The Bug Letter'.
This all dates back to the days of the pre-war Great Western Railway. A lady wrote to the company HQ to complain that she had caught fleas off of the seats in her carriage. The reply was, as it turned out, was what we would call a stereotype reply, something along the lines (excuse pun) of "We are so sorry that you feel that you feel that you have caught an infestation from the seats in one of our railway carriages. We can assure you that we take the matter of cleaning our carriages throroughly. We can only apologise and reassure that the necessary steps will be taken". However, attached to the lady's reply was a small piece of paper which should have been removed by the clerk but had not been..probably starting "Send the old bat" or similar and then "The Bug Letter".

This incident at Mile End happened on Tuesday 17 November 2009 So which party asked for it to be withheld till today? ;D Make your own minds up. Or is the state opening such a non-event this time that the Beeb needed something to beef up the news??

It's quite ironic that a safety modification ended up injuring 3 passengers!
If you count the number of warning notices inside a tube train - you wonder if they actually make you any safer.

The notices aren't there to make you safer. They're there to ensure that, in the event of an incident that was covered by the notices, you would be unable to sue! One wonders if that includes a neck injury due to rough riding of the train sustained while reading a high-level notice inside the coach.... Sometimes I wonder if twenty-odd years on the railways has made me more cynical.

I would not like to get hit by the barrier on the A60 stock. It contains two wooden poles, very close in size to cricket stumps!!

An LU decision to "bury bad news" on the Queen's Speech day, surely not? BBC/ITV local News are more likely to run a story as soon as possible, but I expect the first they knew about it was when contacted by concerned staff, passengers, or more likely the RMT, today.

Should have used some more cello tape It is funny as the BBC news showed the bit that fell off was a plastic strip on the top of the carriage which are often held in place by 100 mile an hour tape. But alas it seems as if that wasn't the problem. Bottom line is no matter how much maintenance you give something, something can always go long. The key is to minimise the impact of something going wrong. Knee jerk reactions often on the call of media or politicians are the end result.
