Are the warning daggers at Charing Cross and Leicester Square keyed on the station poster? If they are to do with walking to Covent Garden, shouldn't Holborn be daggered too, and Temple (most peaceful station, always use it to get there)? Why would someone going to Covent Garden think of looking at these? Why should he/she?
Or is it something to do with the Northern Line split?
Custom House has lost part of its wheelchair, making it look like a very peculiar smiley.

Well spotted, i hadn't noticed! It looks as if they are to do with Covent Garden, as the poster map index for Charing Cross says: "To get to Covent Garden - avoid the crowds by taking a short walk from this station (11minutes)"
Leicester Square says: "To get to Covent Garden - avoid the crowds by taking a short walk from this station (6minutes)" tinyurl.com/boaa29I had noticed about Custom House though!

Having dismantled and fiddled with the Tube map in Adobe Illustrator, I can say that they construct the wheelchair out of individual smart objects that they don't group and fix down, so it's extremely easy to wipe a bit of wheelchair off without noticing! Thought something like that wouldn't get through the proofread, though.

There was a rare January 2009 map that was withdrawn after a few days, due to a missing interchange circle at Notting Hill Gate! What's wrong with the October 2008 map?

I think the map with the missing interchange circle at NHG was January 2008, I think I have at least one copy somewhere. I know I gave Angel Islington a copy.
