I read in the thread titled "York Road" that this station together with Caledonian Road and Earl's Court Piccadilly line platforms were built with step free access to platform level.
Which other deep tube stations were built with step free access but lost it when escalators replaced lifts? Do Green Park's lifts use existing or new shafts? Did Shepherd's Bush's uncompleted new lift use an existing or new shaft?

Very few stations were built with lift shafts that went all the way to the platform level - most would stop one level higher than the platform, then there would be a corridor terminating in a set of steps down to the platform.
To convert them into fully step free access would probably be more hassle than simply driving a new tunnel down.

If they drilled a new lift shaft at Shepherds Bush why was it not brought into use

The problem with restoring lift access is two-fold:
1. Quite often, the new escalator shafts went through the old lift shafts; and/or
2. When escalators rising at an angle replaced vertical lift shafts, the surface entrances were changed (Angel, Bond Street for example).

Hyde Park corner is also a great example of the exit point moving. Furthermore as the tunnels and platforms needed to be kept under roads/away from properties as much as possible and station buildings weren't (obviously) in the middle of the road their lifts used to go down adjacent to the lines/platforms and connect through passageways over one line/platform and then stairs. But I do like the idea of a list of stations that used to have step free access. Did Hyde Park Corner?
