There has been a rear-end collision between two trains on the MBTA Green Line D in Boston. Fox News and Google Groups report several serious injuries, including to the operator of one of the trains. The BBC don't appear to have picked up the story yet. I only know about it because by complete chance I was looking for citations of the word "bustitution" for Wiktionary on google groups and this was the first link. edit: The Boston Globe have more detail and a video.

The train operator was killed with numerous passenger injuries. The Boston trains were really trams running on a reserved right of way.
Also, yesterday, there was a derailment on the Chicago El. It llooks like a train split a switch at a junction. There were passenger injuires there, but the train damage looks fairly limited. The El is heavy rail rapid transit with very short carraiges to accomodate very tight curves. It has had serious maintenance problems for several years. (Sound familiar?)
