Does anybody know what works are taking place and the service pattern?

Depends which day you're asking about as its not the same closures on all 4 days. There's Network Rail work on the Wimbledon Branch, Crossrail work at Whitechapel and I suspect some 4LM signalling stuff dotted about too. It's quite a comprehensive answer to do your simple question justice and so I'm happy to let someone else type it out

I’m assuming the Hammersmith terminators are reversing at West Kensington?

It is rather illogical that TfL's website shows only Saturday and Sunday works when you choose "this weekend" - most people would think of Easter as a 4 day weekend. And considering that closures are different between Friday / Monday and Saturday / Sunday, it raises chances of people getting confused.

I’m assuming the Hammersmith terminators are reversing at West Kensington? and not the Barking-Upminster ones as might be inferred from the previous post!

A selection of photos courtesy of the District line Twitter feed showing the ballast to track slab conversion works around West Brompton and the track replacement works at Wimbledon. Note that the latter location saw LU engineering trains using the Up Main Slow Line.
