Does anyone remember the failed push out between Bethnal Green and Liverpool Street in the last years of 62TS?Passengers were trapped for over 2 hours between stations.Two questions 1.What did the two train crews do wrong? 2.How could a simple phone call have avoided their error?

I don't know anything about this incident, but would hazard a guess at the tight bends of the Central line being involved.

I can think of half a dozen things:
- assisting train crew forgot to cut out tripcock - a full coupling was performed and the fault propagated - handbrakes were applied on one of the trains and not released - pilot lighting was lost on the assisting unit - broken train crew didn't advise assisting train of road state ahead - line controller didn't know about it

You're almost right,the crews coupled up two "A" end units together without presspahns so,when they tried to move the control wire on the pushing train applied the EP brake on the defective train.Eventually they burnt out the resistances on the assisting train hence the reports of fire in the tunnel.The simple phone call was when I first went to Hainault as a chargehand examiner,it was drummed into me if there are any trains running in service wrong way round,the Line Controller must be informed in case of any push outs.

They're pieces of non coductive material that clip on the side of the auto-coupler and stop the coupler studs meeting,they don't actually stop the dutch ovens opening.

It's basically a piece of treated cardboard that prevents the studs from making a connection. So yes, in essence the trains will only be mechanically coupled.
